Complaint Form

In order for the board to address issues in the community, we need to have a record of an official complaint from a community member. It is required that all complaints have a witness associated who can confirm they personally witnessed the violation. This form will also provide a clear record of when the request was submitted to provide a timely response. We also encourage sending photo attachments when needed to info@shawneelake.org, currently this form does not support uploading attachments.
